Under local anesthesia, Dr. Velkovski makes a small incision in the gum tissue near the affected tooth to access the end of the root. The infected tissue and the very tip of the root are carefully removed, and a small biocompatible filling is placed to seal the root end and prevent further bacterial entry. The gum tissue is sutured closed and heals over the following weeks. Most patients return to normal activity the next day with minor soreness that resolves quickly.

An apicoectomy is typically recommended when a tooth has had previous root canal treatment but still shows signs of infection on imaging – such as a persistent lesion at the root tip – and when retreating the canal from the top is not practical due to a post, crown, or anatomical complexity.
